چکیده

Coronary artery dissection is a quite unusual but eventually lifethreatening complication, and most cases occur as an immediate adverse event after cardiac catheterization [1, 2]. Mostly, it affects the right coronary artery, due to its anatomical disposition [3]. Rarely it can affect the LMCA (1% of all procedures), as happened in the case reported, and it can have fatal consequences if the adequate management fails to be administered. Usual treatment comprises not only percutaneous intervention, but also surgical myocardial revascularization as long as patient’s haemodynamic stability can be achieved. But not only interventional procedures can be the cause of this complication. It has also been reported to occur in the scenario of a cardiac surgery procedure, despite being a much less frequent complication [4]. In the absence of any other explanation, the cause was assumed to be endothelial lesion due to the direct antegrade cannulation of the coronary ostia, for cardioplegic perfusion. Its clinical presentation is usually intraoperative or immediate post-operative ischemia, when a rapid management and usually surgical treatment is required, but it can also show up even more than a week after surgery, becoming a very unusual complication that should be considered in these patients, when they have a post-operative acute coronary syndrome.

Iatrogenic acute aortic dissection induced by off-pump coronary artery bypass grifting

RATIONALE Iatrogenic acute aortic dissection (IAAD) induced by cardiac surgery is a fatal complication, with 0.04% of therapeutic procedures and worse outcomes than spontaneous aortic dissection. PATIENTS CONCERNS A 64-year-old male complaining of intermittent chest tightness for 4 years received an off-pump coronary artery bypass grifting (OPCABG) and IAAD was found during surgery. DIAGNOS...

Acute coronary syndrome as a result of left main coronary artery stenosis after aortic valve replacement. A report of three cases

Acute coronary syndrome (ACS) as a result of iatrogenic coronary ostial stenosis (ICOS) is a rare but potentially life-threatening complication of aortic valve replacement (AVR). We present three cases of patients with ACS shortly after AVR, in whom ICOS were revealed. They refused an operation and thus they were treated with percutaneous coronary intervention. The potential pathomechanisms of ...

Iatrogenic Ascending Aorta Dissection during Diagnostic Coronary Angiography: Rare but Life-Threatening

Dissection of the ascending aorta is a very rare but life-threatening complication during diagnostic angiography. We present a case of an elderly woman who underwent an elective diagnostic coronary angiography, complicated with an iatrogenic ascending aorta dissection that did not involve the coronary arteries but originated 4 cm distal of the aortic valve. The patient developed cardiogenic sho...

Successful Repair of Type a Aortic Dissection in a Term Pregnancy: A Case Report

Stanford type A acute aortic dissection (AAD) is a very rare complication, with potentially lethal consequences in pregnancy. In fact, pregnancy has been regularly associated with the possibility of aortic. dissection in almost half of young women. Herein, we present the case of a 38-year-old woman in her 37th week of pregnancy. The patient’s medical history was indicative of G4L2Ab1(4 gestaiti...

Resolution of an Acute Aortic Syndrome with Aortic Valve Insufficiency Post-PCI

BACKGROUND Acute aortic syndrome is the modern term that includes aortic dissection, intramural hematoma, and symptomatic aortic ulcer. Iatrogenic coronary artery dissection extending to the aorta during percutaneous coronary intervention is a very rare but life-threatening complication. Despite some reports of spontaneous recovery, most of these patients are treated surgically as a spontaneous...
